Parents/carers must complete an emergency medical treatment form before a member of staff can administer any form of emergency medical treatment.

All staff are trained to administer use of the Epi-pen and will administer asthma medication. Parents must agree to any other medical treatment.

There will always be a fully trained First Aider on site to administer or advise the treatment.

Children with diarrhea, vomiting or any other contagious illness will not be permitted at the nursery 24 hours after the last episode.

The following form is to be completed and signed by the parent(s)/carer(s) and returned to the school.
